Guidance Notes
There are 3 questionnaires and it is hoped that you will complete each questionnaire when you have come to the end of each stage in the process as follows:
Questionnaire 1: DSA-QAG Assessment Centre Student Survey
Questionnaire 1 will allow you to comment, provide feedback and rate the service provided by the centre you attended to undergo an assessment as part of the completion of your Needs Assessment Report.
Note: You should complete this when you have agreed / authorised your Needs Assessment Report and it has been sent for approval to the funding body
Questionnaire 2: DSA-QAG Supplier Student Survey
Questionnaire 2 will allow you to comment, provide feedback and rate the service provided by the supplier who has provided the hardware and software agreed in your assessment report.
Note: You should complete this part of the survey when your hardware, software and any equipment awareness training has been delivered and undertaken.
Questionnaire stage 3: DSA-QAG On-going Support Student Survey
Questionnaire 3 will allow you to comment, provide feedback and rate the service provided by the supplier providing the on-going support for your equipment during the duration of your course. This includes the helpdesk service provided along with repair timescales etc.
Note: You should complete this part of the survey if you have had an occasion to use this service at any point during your course.
Completing the Survey
To allow QAG to validate participants in the survey we are asking you to enter your student ID reference number. We have included a field for your name and email address to be entered, this is an optional field rather than a mandatory field, however if you do want to be entered into the draw for the iPod Touch then you should enter your name and email address.
